KIT, DENGUE/CHIKUNGUNYA , 10 000 people
Definition
Medical and logistic support, sent to an area where a Dengue or Chikungunya outbreak is expected or ongoing. The kit allows to implement vector control activities within and around 5 medical infrastructures and for a total of 2000 families for the first month of intervention. Local purchase is not considered during the first phase of the emergency.
Chikungunya is a viral disease that is spread by mosquitoes: Aedes aegypti and Aedes albopictus, two species which can also transmit other mosquito-borne viruses, including dengue. The disease shares some clinical signs with dengue. It causes fever and severe joint pain.
The kit can also be used in a yellow fever outbreak where timely availability of vaccines is an issue.
Indications
This kit is specially designed to implement targeted vector control activities. It can be used within and around specific infrastructures (medical infrastructures, sites where breeding sites are identified), in refugee camps and as well for a targeted urban or rural population.
Components
It is composed of an entomological assessment module, a module containing vector control equipment and a module with insecticides.
- Entomological assessment: the reduction of the number of breeding sites of the mosquito by sanitation (elimination of areas with stagnant water and waste) and the promotion of health seeking behaviours to reduce all non protected water reserves, play a key role in dengue control.
- Module containing vector control equipment, as well as personal protective equipment for the operators.
- Module containing insecticides (which are adulticides for IRS, larvicide, adulticide + additives for fumigation) and insecticides (Etofenprox = CWATINSEIE1L) for the impregnation of clothing (see "Public Health Engineering in Precarious Situations", MSF, 2nd edition, 2010, chapter T.B. 7.17 Impregnation of nets, clothing and curtains).
Three modules are necessary to implement an entomological survey in the first phase of the emergency since the identification of the breeding sites will allow an adapted, rapid and targeted vector control intervention.
Links to exisiting rapid diagnostic tests are given to enable the initial diagnostic process.
Caution
All equipment and insecticides complies with the WHO specifications (WHO/CDS/RBM/ 2001.28).
Instructions for use
MSF is increasingly confronted with Dengue outbreaks. With no appropriate vaccine or treatment available, an adapted vector control should be considered to decrease the extend of the epidemic.
This as per recommendations in the WHO guide: Prevention and Control of Dengue and Dengue Haemorrhagic Fever. Comprehensive guidelines, WHO, SEARO n° 29, 1999.
Precautions for Use
- Store insecticides in appropriate conditions (e.g. avoid heat, direct sunlight)
- The staff implementing the vector control activities must be properly trained and fully protected (e.g. earplugs for staff operating thermofoggers).
Storage
The insecticides included in the kit have a limited lifetime as indicated.
