CL Test trypanosomiasis (CATT)

Used Techniques

Rapid serological test (CATT test) used for the screening of Trypanosoma gambiense (sleeping sickness).

Instructions for use

The CATT is the only screening test for trypanosomiasis. The diagnosis of trypanosomiasis must be completed with other tests:

  • liquid of ganglionary puncture
  • Woo test: looking for trypanosoma in a blood sample
  • lumbar puncture (double centrifuge method) to determine the stage of the disease

Instructions for use

The Giemsa solution should be extemporaneously prepared. The racks for staining and drying slides can be locally manufactured.

Malaria, filariasis and borreliasis are generally done with a capillary blood sample (we can also use venous blood). Do not forget equipment for blood sample collection.

Leishmaniasis needs an lymph node or spleen puncture, what requires a particular training which is organized only in some specific programmes.

CL drepanocytosis, Emmel test

Used Techniques

Emmel test (= sickle cell test) on whole blood/ capillary blood with sodium disulfite (metabisulfite) or saline.

Instructions for use

The technique with sodium disulfite is recommended as it is more sensitive than saline.

A scale is needed to prepare the reagent (2% solution: 0.5 g for 25 ml distilled water).

The metabisulphite solution is unstable and can be kept maximum 8 hours.

Alternative: test with paraffine.

It is recommended to wear gloves during the test.

The MTB/RIF cartridge will be discontinued by the manufacturer in December 2023 and replaced by the MTB/RIF Ultra cartridge.

Definition

The GeneXpert is an automated molecular cartridge based, closed system to perform nucleic acid amplification testing (NAAT).

The test Xpert MTB/RIF and MTB/RIF Ultra cartridge is used to detect Mycobacterium tuberculosis and rifampicine resistance. The MTB/RIF Ultra has a lower limit of detection than MTB/RIF, reduces false rifampicin resistance and better differentiate silent from resistant-conferring mutations.

The test Xpert MTB/XDR is used to detect extensively drug resistant Mycobacterium tuberculosis, with resistance detection to isoniazid, ethionamide, fluoroquinolone, second line injectable drug.

Instructions for use

The GeneXpert® MTB test does not eliminate the need for conventional microscopy, culture and DST, which are required to monitor treatment progress and to detect resistance of Mycobacterium tuberculosis to drugs.

The operating temperature for GeneXpert device is 15-30°C.

CL GeneXpert EID

Definition

The GeneXpert is an automated molecular cartridge based, closed system to perform nucleic acid amplification testing (NAAT). One of the two cartridges currently being used, is the Xpert HIV-1 Qual EID for the qualitative in vitro detection of HIV-1 total nucleic acids using whole blood and dried blood spot. In MSF programs this test shall be used for early infant diagnosis of HIV-1.

The Xpert® HIV-1 Qual assay is not intended to be used as screening test of blood donors for HIV-1.

Instructions for use

  • The operating temperature for GeneXpert device is 15-30°C.
  • Standard safety precautions for handling human specimens should be followed when carrying out this test.
  • The Eppendorf Thermomixer and Eppendorf Smartblock in the checklist are only needed for the DBS application.

As communicated internally previously (LWG, December 2018), the TI transport medium coming from Oslo has always been meant to be used ONLY for the purpose of global surveillance, and not for any other purpose. For any other testing purpose, TI transport medium needs to be requested at country level from the national referral laboratory/public health institute or regional WHO representative. Realizing the complexity of this supply, the MSF laboratory working group is investigating possible alternative supply sources, but this has proven complicated in the past and is unlikely to provide solutions in near future.

Used Techniques

Thin / thick blood film stained with Giemsa for the detection of malaria, borreliasis, leishmaniasis, filariasis.

Instructions for use

The Giemsa solution should be extemporaneously prepared. The racks for staining and drying slides can be locally manufactured.

Malaria, filariasis and borreliasis are generally done with a capillary blood sample (we can also use venous blood). Do not forget equipment for blood sample collection.

It is recommended to wear gloves for any handling of blood samples.

Leishmaniasis needs an lymph node or spleen puncture, what requires a particular training which is organized only in some specific programmes.

Visceral leishmaniasis (Kala-Azar) requires in addition another diagnostic method with a serological test.
