Patient booklet in French included in the ATFC protocol (ambulatory therapeutic feeding programme).
Health card for caretaker of the child. Used for documenting the weight/MUAC and amount of RUTF given as well as the next appointment date to return to the ATFC. The caretaker brings the booklet to each ATFC visit. Details of systematic treatment given and discharge details are also recorded.
The caretaker is advised to take it with him to future medical appointments or if the child is readmitted to the CNTA, to keep a record of previous admissions and treatments.
