This article can not be ordered via the MSF supply centres. Limited amount is distributed yearly in the countries of the Meningitis belt for surveillance purpose only (see more information below)

Definition

Trans Isolate (TI) medium supports the growth and survival of Neisseria meningitides, Streptococcus pneumoniae and Haemophilus influenza.

MSF has an agreement with the Norwegian Public Health Institute in Oslo, WHO Collaborating Centre for Reference and Research on Meningococci, to support its role in following the evolution of meningitis strains.

As part of this agreement, the Norwegian Public Health Institute is providing the TI medium free of charge to MSF for strain identification alone.

Instructions for use

  • Usage of TI transport medium for CSF distributed by MSF is validated only for Meningitis outbreak investigation in the Meningitis belt[i], NOT for individual testing (routine CSF culture) and only for MSF projects.
  • MSF should follow the Meningitis outbreak response plan by the Ministry of Health (MOH). This plan should include the procedures for confirmation in the event of an outbreak.
  • One or two TI tubes should be prepared per CSF sample at the beginning of each outbreak. CSF sample will be sent to the following:
    • Norwegian Public Health Institute (Oslo) to perform strain identification but not confirmation of outbreak (as they receive only a subset of samples and perform the analysis at a later stage)
    • Reference laboratory for confirmation of outbreak – this will be country dependent, depending on the functionality of the culture laboratory and according to MOH procedures
  • MSF should discuss and agree with the MOH the procedure for sending samples of each Meningitis outbreak to the Norwegian Public Health Institute (Oslo) for strain surveillance by culture and molecular techniques.
  • For uses outside of the agreement with the Norwegian Public Health Institute (Oslo) – such as individual testing and/or use outside meningitis belt region, TI should be procured through WHO or MOH (country level) as it is NOT available through MSF supply centers.

[i]The 14 countries in the African Meningitis Belt with enhanced surveillance for meningococcal disease include Benin, Burkina Faso, Cameroon, the Central African Republic, Chad, Côte d'Ivoire, the Democratic Republic of the Congo, Ethiopia, Ghana, Mali, Niger, Nigeria, Sudan and Togo.

Storage

Shelf life: 12 months

The shelf life of the TI (STSSTRTUTI1, produced by Oslo) is 1 year. However, the medium can be used as long as no growth is observed in the liquid (remains clear without growth) or solid phase (no growth of colonies).

Important: The extension of shelf life is an exceptional measure, validated by the manufacturer (e.g. the Norvegian Public Health Institute) and not to be applied on any other items.

MSF requirements

  • The Trans Isolate can not be ordered through MSF supply centers. It is distributed automatically once a year as EPREP before the start of the Meningitis season in countries in the Meningitis Belt.
