Used Techniques
Thin / thick blood film stained with Giemsa for the detection of malaria, borreliasis, leishmaniasis, filariasis.
Instructions for use
The Giemsa solution should be extemporaneously prepared. The racks for staining and drying slides can be locally manufactured.
Malaria, filariasis and borreliasis are generally done with a capillary blood sample (we can also use venous blood). Do not forget equipment for blood sample collection.
It is recommended to wear gloves for any handling of blood samples.
Leishmaniasis needs an lymph node or spleen puncture, what requires a particular training which is organized only in some specific programmes.
Visceral leishmaniasis (Kala-Azar) requires in addition another diagnostic method with a serological test.

The MTB/RIF cartridge will be discontinued by the manufacturer in December 2023 and replaced by the MTB/RIF Ultra cartridge.
Definition
The GeneXpert is an automated molecular cartridge based, closed system to perform nucleic acid amplification testing (NAAT).
The test Xpert MTB/RIF and MTB/RIF Ultra cartridge is used to detect Mycobacterium tuberculosis and rifampicine resistance. The MTB/RIF Ultra has a lower limit of detection than MTB/RIF, reduces false rifampicin resistance and better differentiate silent from resistant-conferring mutations.
The test Xpert MTB/XDR is used to detect extensively drug resistant Mycobacterium tuberculosis, with resistance detection to isoniazid, ethionamide, fluoroquinolone, second line injectable drug.
Instructions for use
The GeneXpert® MTB test does not eliminate the need for conventional microscopy, culture and DST, which are required to monitor treatment progress and to detect resistance of Mycobacterium tuberculosis to drugs.
The operating temperature for GeneXpert device is 15-30°C.

Definition
The SIGN standard nail was designed and began use in 1999 and is a solid stainless steel nail with a 9° proximal bend and 1.5° distal bend, offered in 8 mm to 12 mm diameters and 280 mm to 420 mm lengths. It is an intramedullary nail that is held in place by interlocking screws to stabilize a fractured bone and enable bone healing.
SIGN Nails are held in place with interlocking screws. These screws are inserted through the slots of the nail to stabilize the fractured bone segments. Screws are manufactured in a range of lengths and diameters to fit the bone and fracture type. Depending on the type of fracture and SIGN Nail used, 1 to 4 screws can be used in each surgery.
Definition
Instruments and equipment needed for the placing of the SIGN IM nail. All instruments are non sterile and reusable, they have to be sterilised prior to use.
Instructions for use
SIGN's Target Arm and Slot Finder work together, enabling a surgeon to locate the distal slot in the nail and insert an interlocking screw with tactile feedback, rather than relying on C-arm imaging.

Line probe assays (LPA) are a family of DNA strip-based tests that allow users to determine the drug resistance profile of an MTB strain by interpreting a pattern of bands that represent lines of immobilized probes that are bound (or hybridized) to MTB amplicons (DNA amplification products). LPA probes are designed to target the most common mutations associated with resistance to first- and second-line anti-TB agents and specific MTBC wild-type (WT) DNA sequences.
This checklist details a set of materials needed for the drug susceptibility testing of M. tuberculosis and the differentiation between M. tuberculosis and 27 nontuberculous Mycobacteria;
- GenoType MTBDRplus VER 2.0: M. tuberculosis complex and its resistances to rifampicin and isoniazid from specimens or cultures
- GenoType MTBDRsl VER 2.0: M. tuberculosis complex and its resistances to fluoroquinolones and injectable antibiotics (kanamycin, amikacin, capreomycin, and viomycin) from specimens or cultures
- GenoType Mycobacterium CM VER 2.0: Detection of M. tuberculosis complex and 27 clinically relevant NTM from cultures
Instructions for use
Mycobacterial DNA is extracted from sputum specimens or cultivated material, specifically amplified via PCR and detected on a membrane strip using reverse hybridization and an enzymatic colour reaction. Valid results are documented by internal controls, Conjugate and Amplification Control.
Molecular testing requires specific laboratory set up, equipment and trained personnel. Refer to your lab advisor.
HAIN LPA does not eliminate the need for conventional microscopy, culture and DST, which are required to monitor treatment success and to assess resistance to other drugs.
Standard safety precautions for handling human specimens should be followed when carrying out this test. Always wear suitable protective clothing and gloves. Handling of potentially infectious specimens must be carried out in a class II safety cabinet.

Definition
List of necessary items for the collection of CSF and transport to the reference laboratory using transport medium for CSF (Trans Isolate) medium in case of suspicion of a meningitis outbreak in the African meningitis belt [i].
Instructions for use
The Trans Isolate (TI) medium supports the growth and survival of Neisseria meningitides, Streptococcus pneumoniae and Haemophilus influenza. MSF has an agreement with the Norwegian Public Health Institute in Oslo, WHO Collaborating Centre for Reference and Research on Meningococci, to support its role in the surveillance of the evolution of meningitis strains. As part of this agreement, the Norwegian Public Health Institute is providing the TI medium free of charge to MSF for strain identification alone.
In 2023, MSF is only getting 120 bottles of the TI transport medium
from the supplier is the Norwegian Public Health Institute (Oslo).
As communicated internally previously (LWG, December 2018), the TI transport medium coming from Oslo has always been meant to be used ONLY for the purpose of global surveillance, and not for any other purpose. For any other testing purpose, TI transport medium needs to be requested at country level from the national referral laboratory/public health institute or regional WHO representative. Realizing the complexity of this supply, the MSF laboratory working group is investigating possible alternative supply sources, but this has proven complicated in the past and is unlikely to provide solutions in near future.
The pragmatic decision was made that the 120 TI bottles will be distributed equally to 12 different MSF missions within the African meningitis belt , and that in each country they will be prepositioned in 1 coordination office, available for all sections to use for the specific purpose of sending CSF samples to Oslo only.
[i] The 14 countries in the African Meningitis Belt with enhanced surveillance for meningococcal disease include Benin, Burkina Faso, Cameroon, the Central African Republic, Chad, Côte d'Ivoire, the Democratic Republic of the Congo, Ethiopia, Ghana, Mali, Niger, Nigeria, Sudan and Togo
Other remarks for use:
- Do not forget material for CSF collection
- Always wear gloves to manipulate samples

Leishmaniasis needs an lymph node or spleen puncture, what requires a particular training which is organized only in some specific programmes.
Visceral leishmaniasis (Kala-Azar) requires in addition another diagnostic method with a serological test.
